  2. Home Safety Matters – Is your home fall-proof? Are there grab bars or walking aids if needed?
  3. Who’s Checking In? – Have a plan for who’s helping in the first few days—family, a carer, or a community service?
  4. Watch for Warning Signs – Know which symptoms mean you should seek help fast.
  5. Connect with Support Services – Transitional care, My Aged Care, CHSP—help is available, but you need to know how to access it.
